No. Previews stay in your browser. The downloaded and copied code is a real form that posts to the endpoint you paste in.
Yes. Open a template in the visual designer to add, remove and reorder fields, or edit the HTML directly. Keep the anti-spam field and give every input a name.
All templates work on the free plan. Templates with a file upload field, such as the job application and bug report forms, need uploads enabled, which is part of Pro.
